   Any x-rays needed
   5% off implant-related treatment
   20% off any other treatment when needed

• The Fee listed for plans is cash/check price. For credit card payments a 3% surcharge will be added.
• Once purchased, plans are non-refundable.
• Plans are not transferrable.
• It is the patient’s responsibility to utilize the benefits of the plan during the term of the plan (1yr from date of purchase)
• Care Credit is not accepted under discount plans.
